This privacy policy explains how Southside Physiotherapy Clinic collects and uses your personal information.

What personal information we collect
We collect information relevant to the treatment offered by us i.e. your name, address, DOB, medical history, medication, physical activities pursued and a history relating to the specific reason for attending the clinic. We also document the findings from the physical assessment we carry out. This allows us to provide appropriate treatment and advise in relation to your specific diagnosis.

Where we collect personal information from
We collect information from you in person or via mail, telephone, email, text or the ‘contact us’ page on our website. Also, with your permission, we collect information from your employer (if relevant), insurer and other health professional or clinical adviser and understand that any insurer would only refer you with your express consent.

Where we store your personal information
All clinical records and scanned related communication from medical professionals and insurance companies are stored on a physiotherapy-specific data base supplied by TM2/Blue Zinc IT Ltd.

How we use your personal information

We use your personal information to
  • deliver our services to you and meet legal responsibilities
  • communicate by post, email or telephone regarding your treatment, which may include quality feedback.
  • maintain clinical records
  • carry out audit

Who we share your personal information with
We share your personal information with your General Practitioner and other third parties who you request us to share it with, for example your Insurer. We do NOT pass your details to anyone else.

If you do not provide your personal information
If you do not provide us with the personal information required, we may be unable to deliver our services to you.

How long we retain your personal information for
Information will be kept for as long as necessary to meet regulatory requirements. In determining retention periods, reference will be made to guidance issued by the Chartered Society Physiotherapy and HCPC (Health Professions and Care Council), our professional and regulatory bodies.

Your rights
Access to your information — You have the right to request a copy of personal information that we hold at any time.
Correcting your information — We want to make sure that personal information is accurate, complete and up to date, and you may ask us to correct any personal information that you believe does not meet these standards at any time. 
Restricting how we may use your information —You may ask us to restrict how we use personal information for communication, and we will only use the relevant personal information with your consent.
